In a bid to provide enhanced healthcare accessibility and affordability to Singaporeans, the Ministry of Health has announced significant updates to the Healthier SG Chronic Tier subsidy framework, set to be implemented starting February 1, 2024. This initiative marks a pivotal step forward in ensuring that eligible individuals receive comprehensive support for managing chronic conditions through their enrolled Healthier SG General Practitioner (GP) clinics.
Understanding the Subsidy Framework
Under the Healthier SG Chronic Tier subsidy framework, Community Health Assist Scheme (CHAS), Pioneer Generation (PG), and Merdeka Generation (MG) cardholders will gain access to selected medications for chronic conditions at their enrolled Healthier SG GP clinics. These medications, essential for managing conditions under the Chronic Disease Management Programme (CDMP), will be available at prices comparable to those offered at polyclinics.
Enhanced Subsidies
Eligible cardholders stand to benefit from means-tested subsidies of up to 87.5% for selected chronic medications, with no dollar cap at their enrolled GP clinic. Additionally, subsidies of up to $360 per year will be extended for other components of their care, including consultation, lab tests, and additional medications.

Choice and Flexibility
Patients will have the flexibility to choose between the existing CHAS Chronic Tier and the new Healthier SG Chronic Tier at each visit, allowing them to apply the subsidy tier that best suits their medication needs. Pro-ration of the annual dollar subsidy balance will be applied accordingly.
Medication Accessibility
Patients can collect their medications directly from the clinic or opt for medication delivery, with a nominal charge of $4 per delivery (only for medications from the Healthier SG Medication List). It is advised that patients confirm the details with their clinic.
Use of MediSave
Enrolled residents with chronic conditions under the CDMP will have the option to use MediSave to fully pay for their treatment at their enrolled Healthier SG clinics, up to the MediSave500/700 withdrawal limit. This eliminates the need for a 15% cash co-payment, enhancing financial accessibility for patients.
Enrolment and Accessibility
