CDMP Chronic Conditions under Healthier SG and CHAS Subsidies
Caring for chronic conditions is one of the pillars of Healthier SG. There is a list of 23 conditions recognised as chronic conditions under the Chronic Diseases Management Programme ( CDMP), as below
- Diabetes Mellitus/ Pre-diabetes
- Hypertension
- Hyperlipidemia (Lipid Disorders)
- Stroke
- Asthma
- Chronic Obstructive Pulmonary Disease (COPD)
- Schizophrenia
- Major Depression
- Bipolar Disorder
- Dementia
- Osteoarthritis
- Benign Prostatic Hyperplasia
- Anxiety
- Parkinson’s Disease
- Chronic Kidney Disease (Nephrosis/Nephritis)
- Epilepsy
- Osteoporosis
- Psoriasis
- Rheumatoid Arthritis
- Ischaemic Heart Disease
- Allergic Rhinitis
- Gout
- Chronic Hepatitis B
If you have any of these conditions, you are eligible for subsidies for chronic medications according to the chart below:
To navigate this table, first identify the tier of CHAS Card you hold e.g Green or Orange or blue, or if you are a Merdeka and Pioneer Generation Card holder.
Then, identify if you are considered a Simple or Complex case. This is decided based on the number of chronic conditions you have in the list below. If you have one condition, you are under Simple. If you have two or more, you are under Complex.
For example, A Merdeka Generation card holder with Hypertension and Diabetes ( Complex) would be eligible for $520 of subsidy per year for their medications.

Additional Medisave Subsidies ( MediSave 500/700 scheme)
On top of CHAS subsidies, you can also utilise your medisave to pay for chronic disease management, up to $500 (for Simple) and $700 (for Complex) per annum. There is NO cash copayment if utilising medisave at your Healthier SG enrolled clinic (Usually 15% cash copayment at non-enrolled clinic).
This can also be used to pay for certain investigations such as ultrasound scans that may be necessary in management of your chronic conditions. Do speak to our doctors to find out more.
