What standard liver panels miss

ALT and AST are late signals. By the time they rise, the liver is already under stress.

A standard liver panel — included in most health screens in Singapore, including Screen for Life — measures liver enzymes (ALT, AST, GGT), albumin, and bilirubin. These are useful markers. But they have a critical limitation.

ALT and AST measure liver cell damage — enzymes that leak into the blood when hepatocytes (liver cells) are injured. They can remain within normal limits even when 20–30% of the liver is fatty. GGT is more sensitive to alcohol — it is essentially a biological dosimeter for regular drinking — but standard lab reports rarely explain what a trending GGT actually means in the context of your drinking pattern.

What standard panels do not assess: whether fibrosis is developing (FIB-4, calculated from your existing blood results, has a 90% negative predictive value for ruling out advanced fibrosis — yet is almost never calculated on a standard report), or whether insulin resistance is driving metabolic stress on the liver (HOMA-IR). Imaging — FibroScan or ultrasound — can add further clarity where clinically appropriate. These are the markers that tell you where your liver is heading — not just where it is today.

Frequently Asked Questions

Can fatty liver be reversed?
Yes — in early stages, fatty liver is reversible through sustained lifestyle changes: reduced alcohol intake, improved diet, regular exercise, and targeted weight loss. The key is catching it early, before it progresses to fibrosis. This is why screening matters: by the time symptoms appear, the window for easy reversal may have narrowed significantly.
Is a normal GGT result reassuring?
Not necessarily. GGT is the most alcohol-sensitive liver enzyme and often the first to rise with regular drinking. However, standard lab reference ranges are wide — a GGT that’s “within normal limits” may still be elevated relative to your baseline or trending upward. A GGT of 45 in someone who drinks 3–4 times per week tells a different story than a GGT of 45 in someone who rarely drinks. Context matters — and that’s what the Liver Composite Score provides.
What is the Liver Composite Score?
AMG’s structured assessment of liver health across four dimensions: liver enzymes (ALT, AST, GGT), fibrosis risk (FIB-4, liver stiffness where available), metabolic drivers (HOMA-IR, visceral fat, ferritin), and imaging (abdominal ultrasound). Unlike a standard liver panel that reports individual numbers, the Composite Score interprets them together.
What does a standard liver panel actually test?
A standard liver panel measures ALT, AST, GGT, albumin, and bilirubin. These are useful — but ALT and AST can remain within normal limits even when fatty liver is present. Standard panels do not include fibrosis risk (FIB-4), insulin resistance (HOMA-IR), or liver imaging. The Liver Composite Score includes all of these.
