Statins are widely prescribed under schemes like CHAS, MediSave, or MediShield Life. They work by blocking cholesterol production in the liver and are effective in most patients.
However, some people experience side effects like muscle pain or liver enzyme abnormalities—or do not achieve target LDL levels despite maximum tolerated doses.
PCSK9 Inhibitors: A Game-Changer in Cholesterol Therapy
PCSK9 inhibitors are injectable biologic medications that reduce LDL cholesterol by enhancing the liver’s ability to clear it from the bloodstream. They target a protein called proprotein convertase subtilisin/kexin type 9 (PCSK9) which, when inhibited, increases the number of LDL receptors available to remove cholesterol.
Available in Singapore:
- Evolocumab (Repatha®)
- Alirocumab (Praluent®)
These drugs are typically administered every 2–4 weeks via subcutaneous injection.
*Repatha is currently in the Healthier SG Medication List, HSG patients may receive additional government subsidies for this medication.
Who Should Consider PCSK9 Inhibitors?
Your doctor may recommend PCSK9 inhibitors if you:
- Have familial hypercholesterolaemia (FH), a genetic condition with very high LDL from a young age
- Have had a heart attack, stroke, or other cardiovascular event
- Are intolerant to statins
- Still have elevated LDL despite maximally tolerated statin therapy

Clinical Evidence: How Effective Are PCSK9 Inhibitors?
Several landmark studies have demonstrated the power of PCSK9 inhibitors:
- FOURIER Trial (Sabatine et al., 2017, New England Journal of Medicine):
- Evolocumab reduced LDL by 59% and lowered the risk of major cardiovascular events by 15%.

- ODYSSEY OUTCOMES Trial (Schwartz et al., 2018, NEJM):

- Medication Assistance Fund (MAF)
- Means testing via public hospitals
- Private insurance plans covering high-risk individuals
